Freedom of Information

Submit a request

The Trust fully supports a culture of honesty and openness, and fully complies with the Freedom of Information (FOI) Act 2000.

You can submit an FOI request to view information held by us to find out how we are providing services, and the standards and targets we are meeting.

Please note that the Act does not however cover access to your personal data. If you would like to see information such as your health records, please see accessing your Medical Records page for more information.

Freedom of Information requests

We hope you will find what you are looking for on our website, but if not, you can send your Freedom of Information (FOI) request to us by email or by post.

Please remember to include your full name and address, along with a clear description of the information you are looking for. The FOI team will respond within 20 working days.

Please do not contact our FOI team to request medical records.

If you have any questions, please do not hesitate to contact the team by email. If you are unhappy in any way regarding the manner in which your request has been handled, the Trust has an internal review procedure through which you can raise any concerns you might have.

If you are dissatisfied with the outcome of the complaints procedure, you can apply to the Information Commissioner, who will consider whether the Trust has complied with its obligations under the FOIA, and can require the Trust to remedy any problems.
